Two hours after a prolonged second stage and an oxytocin-augmented labour, a client saturates a perineal pad in fifteen minutes. Six items from cue to reassessment.
1415. Client is a 29-year-old, gravida 3 para 3, 2 hours after a spontaneous vaginal birth of a 4180 g infant. Labour was augmented with oxytocin over 16 hours with a prolonged second stage. Second-degree perineal laceration repaired. Client reports feeling light-headed when helped to the washroom and asks to lie back down. Perineal pad applied at 1400 is fully saturated. A firm, rounded mass is not palpable at the umbilicus; the fundus is felt above and to the right of the umbilicus and feels soft. Bladder palpable above the symphysis pubis. Infant at the breast, latched.
