A 54-year-old client on the surgical unit, day two after an open cholecystectomy, calls the nurse. The client is anxious, reports sudden shortness of breath and sharp right-sided chest pain that worsens on inspiration. SpO₂ is 88% on room air, heart rate 124 bpm, respiratory rate 30 breaths/min, and the right calf is warm and swollen.
